Maple Grove Update

Patient information

Please bring your insurance card with you at your time of visit. You will be asked to fill out a few forms during registration. If you are not the primary holder of your insurance, please come with the primary insurance holder's information, such as: full name, date of birth, social security number and employer name. We also require that you arrange for someone to accompany and drive you home after any surgical procedure that involves sedation.

Directions:

From the South: Take I-94 west to the Maple Grove Parkway exit in Maple Grove. Turn right on Maple Grove Parkway. Turn left on Hospital Drive. Parking lot is on your left

From the North: Take I-94 east to Maple Grove Parkway exit. Turn right onto Maple Grove Parkway. Turn left on Hospital Drive. Parking lot is on your left.
