Discharge Planning
Discharge planning looks at each patient's needs related to:
- Self-care procedures and instructions for the care to be done at home
- Outpatient services if you require additional services for example equipment or therapy
- Home health services for care by professionals delivered in your home
- Long-term care organizations if you need the support of a skilled nursing facility
- Transfer to another hospital for additional treatment if necessary
- Financial information regarding what your insurance company will cover for services you may require
The team of health care providers involved in your discharge plan will include a physician and nurse and could also include a physical therapist, home care nurse, case manager or a social worker. We want to assure that your transition to the next level of care is a smooth and successful process. If you have questions you can ask any member of the health care team.
